Question: Can I see more than one patient at a time for dysphagia therapy, such as during a meal? What code should I use? Can I bill for both individual (one-on-one) and group services provided at separate times to the same patient in the same day?
Answer: Patients can be treated in a group for swallowing. However, there is not a separate code for group swallowing therapy like there is for group speech-language pathology therapy (i.e., 92508). In conversations with officials at CMS (Centers for Medicare and Medicaid Services), ASHA has been told that 92508 is the appropriate code to use for group swallowing therapy. Some facilities/systems may use software designed to screen bills to cross-reference the ICD-9 Code with the CPT code before the bills are submitted to Medicare. ASHA has heard from members that sometimes these systems indicate that 92508 is not a valid code with the ICD-9 code for dysphagia 787.2. If that is happening in your facility or system, you will need to work with your billing office to have this artificial and incorrect “edit” removed from the software.
