Mr. Y is a 90 year-old gentleman who was admitted to a skilled nursing facility (SNF) following AAA repair, CABG x 2, and pacemaker placement. He had a significant history of cardiac dz and CHR Mr. Y had an episode of aspiration pneumonia following emesis after having a trial feeding of pureed food in the acute care facility. A GI specialist also diagnosed severe esophageal dysmotility. When Mr. Y arrived at the SNF, he was NPO with g-tube feeding.
A clinical evaluation revealed a possible swallow delay and apparent reduction in laryngeal elevation. The speech-language pathologist from the acute care hospital had sent the videofluoro-scopic swallow evaluation tapes with the patient. Two examinations had been performed in a 2-month period and revealed significant impairment in pharyngeal and laryngeal motility with aspiration during and after the swallow.
